
Braces vs. Clear Aligners: How We Find the Right Fit for Your Smile
Whether parents walk into our office looking into care for their kids, or adults come in asking about transforming their own smiles, one of the very first questions I hear is: “Should we go with traditional braces or clear aligners?”
It’s a great question! It’s easy to view braces and aligners like picking out items from a store menu, but in my world, they aren’t products—they’re clinical tools. Additionally, tools are only as good as the skill of the practitioner using them!
Thankfully, I’m excellent with either approach—the key then becomes selecting the right “tool” for the job. Braces are better and more appropriate to achieve certain goals, and the same is true for clear aligners. Sometimes, either approach is a totally good option! Sometimes one or the other makes more sense. These are the kinds of questions we dive into during an initial consultation…
My primary job as an orthodontist is to take a close look at your bite, jaw alignment, and tooth positions to figure out which specific approach will give you the healthiest, most stable result possible AND create the best overall daily experience along the way. Once I determine what’s clinically best, we sit down together and figure out how that plan fits into your real, everyday life. We never want to create an expectation we can’t predictably meet.
Here is a look at how I evaluate both options when designing treatment plans for our local families across Southern Indiana.
Traditional Braces: Precise, Practically “Automatic” Progress
First off, throw out whatever picture you have in your head of braces from twenty years ago! Modern brackets are smaller, lower-profile, and significantly more comfortable than what most of us grew up with. They’re more “dialed-in” allowing for the highest degrees of comfort, efficiency, and predictability.
- When I Recommend Them: Certain complex tooth movements, severely rotated teeth, or significant jaw or bite alignment issues simply respond better to the constant, fixed control of brackets and wires. Tooth alignment and most bite corrections are generally MUCH more predictable and efficient with braces than with clear aligners (up to 35-40% in many cases). These facts are proven time and again in a huge body of robust clinical research.
- The Lifestyle Fit: Because braces are attached to your teeth, treatment progress happens automatically 24/7. Parents don’t have to worry about kids losing clear aligner trays in the school cafeteria trash, and busy adults don’t have to keep track of taking trays out during meetings or coffee breaks. Aside from keeping them clean and being careful not to break my stuff, all you have to do is tolerate wearing them. I have maximum control with braces. I’m at the wheel. You’re mostly just along for the ride.
- The Daily Routine: You’ll need to spend a little extra time brushing around brackets and need to avoid extra-sticky or hard foods that can pop a bracket loose. Many patients, especially the younger crowd, usually find picking fun bracket colors for sports seasons or holidays is worth the trade-off!
Clear Aligners: Low-Profile Flexibility
Clear aligners use a custom series of smooth, removable plastic trays to guide teeth into position step-by-step.
- When I Recommend Them: Aligners are fantastic for mild to moderate crowding, spacing, and specific bite corrections where less precise, tray-based pressure works best. Sometimes patients also have legitimate lifestyle or health-related concerns that make wearing braces a bad idea, so clear aligners can be a great alternative. Very importantly, we will discuss the three main differences between clear aligners and braces: 1. Clear aligners are less efficient overall than braces in most cases due to decreased precision, control, and predictability, 2. The amount of day-to-day effort required to properly manage clear aligner progress is higher, and 3. Due to the increased lab expense associated with clear aligner treatments, fees are usually higher.
- The Lifestyle Fit: Because they’re virtually invisible, aligners are popular with adults in professional settings or teens who want the most subtle look. Plus, because they’re removable, you have zero food restrictions—popcorn at high school football games is still on the menu—and normal brushing and flossing are a breeze. If a patient is clear on expectations and up for the responsibility that comes with clear aligners, they can be an awesome option!
- The Daily Routine: Aligners only work if they are in your mouth for 20 to 22 hours every single day. During our consultation, we’ll talk honestly about whether a tray-based system fits your routine without adding extra daily stress. Anyone considering clear aligners MUST be realistic, though: clear aligners are 100% compliance-dependent. They ONLY work if worn as prescribed. Many patients think they’re up for that challenge only to find that such a high expectation for perfect compliance is much harder to achieve than expected. The last thing we want for any of our patients is a bunch of wasted time, effort, and money. Non-compliant aligner cases are some of the most frustrating cases for EVERYONE.
